Soccer Program participation Policy:
1- I, the parent/guardian of the registrant, the below-named player, a minor, agree that the registrant and I will abide by the rules and regulations of the Silicon Valley Eagles Youth Soccer, its affiliated organizations and sponsors.
2- Recognizing the possibility of physical injury associated with soccer and in consideration for the Silicon Valley Eagles YS accepting the registrant for its soccer programs and activities (Programs). This signed statement certifies that my child is medically cleared to attend the Silicon Valley Eagles Youth Soccer Academy activities.
3- This signed statement certifies that my child has a health and dental insurance coverage during the Silicon Valley Eagles Youth Soccer Academy activities.
4- I, for myself and the registrant (player) and our respective heirs, administrators and successors, intending to be legally bound, hereby release, discharge and/or otherwise indemnify the Silicon Valley Eagles YS, its affiliated organizations and sponsors, their employees and associated personnel, the owners and operators or the facilities used for the programs, and their respective directors, officers, employees, agents and representatives from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player's participation in the Programs including, without limitation, player's transportation to/from any Program, which transportation is hereby authorized.
5- I further grant the Silicon Valley Eagles Youth Soccer Academy the right to use the player's name, pictures and /or likeness in printed, broadcast, website and other material concerning the Programs provided such use is related to the player's status as a participant in the Programs.
Cancellation & Refund Policy:
6- After the payment confirmation we will assign your child in the team then send you the invitation to join our club in teamsnap (online manager), then we will provide you with more details about the practice days, times and location.
7- This is a soccer season registration (one season only Fall or Spring) of a period of a semester (5 Months) and NOT a monthly basis program, you have to pay in Full the amount to cover the club expenses (League fees, referees, passcards, insurance, equipment and maintenance, and game fields rental)
8- Coaching fee (≅$80 /month) is not included and it is paid in advance directly to the coach in total for a period of 5 months or in Auto-pay installments within the first week of each month. Parents may request a receipt directly from the coach.
9- Cancellations received within 7 days of registration and before the first training session may be eligible to receive a full refund less $100 service fee.
10- Cancellations received after the first training session is NOT eligible to receive a refund.
11- If your child is sick or does not attend the practices for any reason, NO compensation will be made from league registration or coaching fees for the time missed. Note the payment is for a period of 5 months commitment and can Not be prorated for any reason.
12- In case of refund the amount will be paid after the soccer season program by mailing a check or through Paypal.
Disciplinary Actions & Refund Policy:
13- WARNING: When registering to play any and all SV Eagles/PSG programs you acknowledge and agree to abide by the Parents Code of conduct and the Player Code of Conduct Policies. This is considered your warning as you have read our Parents/guardians code of conduct and Player Code of Conduct.
14- FIRST INCIDENT: The parents will be informed, a warning will be issued. if applicable a meeting will be held to ensure the offense doesn't happen again in the future, and the offender may be already suspended from participating for a minimum of one practice or/and one game, with no refund.
15- SECOND INCIDENT: The offender will be suspended from all SV Eagles programs for up to one month, with no refund.
16- THIRD INCIDENT: The offender will be suspended up to 1 season from ALL SV Eagles programs, with no refund.
17- FIGHTING OFFENSE: The offender will be suspended for up to 1 year from ALL SV Eagles programs, with no refund.
18- ILLEGAL CONDUCT: Any illegal offense will not be tolerated. Illegal conduct will be reported immediately to the police. The offender will be suspended for Life from ALL SV Eagles programs, with no refund.
19- By signing up your child to our soccer program you agree to abide by the above terms and policies.
